Welcome! There are a few important things you should know about our program:

  • Virtual Care

We provide virtual addiction care—also known as telehealth—via video, phone, and messaging in our app.

  • Substance Use Treatment

We offer treatment for opioid and/or alcohol use. Everyone’s goals are different—we offer a wide range of services to help you meet yours.

  • Medications

Our care includes medications for addiction treatment (MAT), such as Suboxone (buprenorphine/naloxone).

Tell us a little more about you.

What’s the best way for us to reach you?

We are careful to respect your privacy. We won't share information about the nature of our services on voicemail or to anyone other than you.

What's a good time for us to call you?

A member of the Boulder team will give you a call to discuss your needs and goals, and get you started with treatment.

Our team will do their best to reach out at your requested time. If you need to talk to someone sooner, you can always call us.

Tell us about your health insurance plan.

Please upload pictures of your insurance card below.

With this info, we'll confirm whether Boulder Care services are covered by your insurance provider, and review estimated costs and payment processes together in your first visit.

We never charge for your first visit so that you have the opportunity to ask questions and decide whether Boulder is right for you.

Please upload photos of the front and back of your insurance card below. If you have multiple insurance providers, use your primary insurance. Make sure that all text and numbers on your card are readable in the images. If the name you included above is different from the name on your card, please make note of that in the box below.

That’s okay. You can include any info you do have about your insurance coverage in the box below.

Please note that if we don’t collect your insurance info before your first visit, it may take longer for us to confirm your coverage and/or delay scheduling future visits.

When you have your card, you can upload pictures of it at www.boulder.care/upload.
